
Saw palmetto is commercially available alone and in combination products such as capsules, gel capsules and tablets. There are over 100 commercial products containing saw palmetto as a single ingredient or as a combination product. In castrated rats receiving testosterone, the three treatments significantly reduced the size of the prostate compared with rats (castrated + testosterone) who received no treatment. Although finasteride produced the greatest effect on prostate size, not statistical difference was observed between any of the three treatments. Anti-inflammatory effects of saw palmetto has also been hypothesized to improve symptoms of BPH (6). An acid, lipophilic palmetto (Talso ®) has proven to prevent the formation of inflammatory prostaglandins which produce and leukotrienes.
Saw Palmetto in a dose of 160 mg or placebo three times daily was administered to 35 elderly men and prostate tissue was collected. The Researchers found that some components of the extract of saw palmetto inhibits nuclear estrogen receptors in the prostate of patients with patients with BPH. Clinically, Permixon ® 160 mg twice daily was superior to placebo in a double-blind trial in 110 men. A statistically significant benefit compared placebo was observed in nocturia, flow rate, residual after the evacuation, self-rating, number of doctors, and dysuria. Compared baseline with both placebo and saw palmetto were beneficial in improving nocturia, but only saw palmetto improved flow rate and then residual evacuation compared to baseline. Headache was the only adverse effect. A comparison study doubleblind Proscar ® (finasteride, a 5a-reductaseinhibitor recipe), 5 mg daily, with Permixon, 160 mg twice daily for 6 months. Both finasteride and improvement of saw palmetto International Prostate Symptom Score and quality of life compared to baseline, no statistical difference between the two treatments.
Another randomized, double-blind, placebo-controlled of saw palmetto for the treatment of urinary tract symptoms has also proved useful in such conditions. These researchers studied 85 men, randomized to receive saw palmetto or placebo for 6 months. The effectiveness was monitored using the I-PSS, a questionnaire on sexual function and flow rate urinal. The results of these studies demonstrated that the symptom score decreased I-PSS (ie, improved) 16.7 to 12.3 in subjects who received the palm American dwarf, whereas the symptom score decreased from 15.8 to 13.6 in the placebo group.
